Mental health Community Care within seven days...
The percentage of acute mental health admitted patient separations that have had post-discharge follow up in the community within seven days. Post-discharge follow up in the community is an important strategy for maintaining health and recovery in the vulnerable period following a hospital stay. The follow up indicated is where this has occurred within the public system, where a clinician has spoken directly with the patient (consumer). Results may be under-reported due to missing linkage data between the multiple hospital and community mental health information systems.
